The Referral Process

Our person-centred referral process is in line with Care Quality Commission (CQC) requirements and is carefully adhered to. 
We are careful to ensure that we properly understand the needs of the individual, their likes and dislikes so that we can be certain that we can provide the right support for them. We welcome referrals from private, statutory and third-sector representatives.

Assessments are carried out by experienced members of our team; you can learn a bit more about our Business Development Director Marcus Van-Hagen and our Operations Manager Natalie Smith below:

What attracted you to working with Transforming Support?

I had long believed that supported living was a key provision for the independence and well-being of many with learning disabilities and /or complex needs. Transforming Support offers genuine quality care, and I felt this matched my ambition to challenge the existing provision and excite the market. There was a real opportunity to change. We continue today. 

What is your experience in your field?

Over 20 years working in the mental health, forensic, CAMHs, Brain Injury and Learning Disability fields.  

What do you enjoy in your role?

Finding the right solution that improves the well-being of the person I am working with. Despite all the challenges we are able to always achieve a smile.

What do you like doing when you are not at work

I am an active member of the Royal Air Force as a reservist and Chair of my local allotment association.

What’s your surprise skill?

Diplomacy.

Natalie Smith

What attracted you to working with Transforming Support?

I was already working as a service manager within the Supported Living Sector for adults with Epilepsy, Learning Disabilities, physical disabilities, and behaviours that challenge. Someone I had worked with previously had been working at Transforming Support for some time when he told me of the opportunities and growth going on within Transforming Support, so I started looking into the company. I met the Area Manager and Registered Manager before joining as a Deputy Manager to open the Croydon service.  

What is your experience in your field?

I started working in Health and Social care in 2011 as a support worker in a supported living house. I then started to progress in my career, so I went to supervisor, floor manager, then Deputy Manager and Service Manager.
I have completed my Level 3 and 5 Diploma’s as well as PETTLS, so I used to complete training for new staff. 

Certified CPI: Safety Intervention trainer.  

What do you enjoy in your role?

No day is the same. I enjoy going to meet people, completing assessments and supporting them on their journey into Transforming Support.  Watching and Supporting the people we support grow in some instances to be then able to live independently in the community. 

What do you like doing when you are not at work

Spending time with the friends and family, especially my 4 year old niece.  

What’s your surprise skill?

I can order a mean takeaway!
